PRC-LUCENA in coordination with the Office of Governor Carmencita O. Reyes, thru the Livelihood Manpower Development and Public Employment Service Office (LMD-PESO) will process renewal of professional identification cards and other PRC services at the Provincial Food Terminal, Morion Arena, Boac, Marinduque on February 8, 2013, 8:00 AM - 5:00 PM, and February 9, 2012, 8:00 AM - 12:00 NN and in DepEd South District Office, Sta. Cruz, Marinduque on February 9, 2013, 1:00-5:00 PM and February 10, 2013, 8:00 AM - 5:00 PM.
Below are the instructions to professionals who will renew their expired professional identification cards.
A. Renewal of Professional Identification Cards
– applicants to bring photocopy of expired ID, two (2) passport size pictures in white background with complete name
B. Change of Name due to Marriage/Correction of Name or Date of Birth
Petition Form and original copy of NSO Birth/Marriage Certificate in security paper
Photocopy of professional ID
Four (4) pieces passport-size picture, white background, with complete name
C. For applications in licensure examinations
1. Submit their applications through our Online Application System (OAS) in any computer with Internet connection. They shall log on to the PRC website….www.prc.gov.ph and click Online Application System icon.
2. Send scanned picture, colored, passport size, in white background with complete name to firstname.lastname@example.org. Indicate in the subject: “Marinduque exam applicant”. Scanned pictures may be sent individually or by group of applicants
3. Upon arrival of the PRC Mobile Team, submit the following documents:
original and photocopies of Transcript of Records, Birth Certificate from NSO on security paper, two (2) passport size pictures in white background with complete name, cedula. Fees to be paid to PRC Team: Examination fee of P900.00 and P35.00 for documentary stamp and mailing costs.
